One mile north of Dixiana east of Route 620 in Wise County. The permit area is on the southern slope of Indian Mountain and extends from Route 620 east to Steve Horn Branch
The permit area is located on the Flat Gap, VA-KY USGS quadrangle map.
The proposed operation will come within 100 feet of State Route 620.
Drainage and wastewater resulting from the permitted operation will pass through approved sediment control structures and be discharged to Guest River and Steve Horn Branch.

TO WHOM IT MAY CONCERN:
The Department of Mines, Minerals, and Energy, Division of Mined Land Reclamation, of the Commonwealth of Virginia, pursuant to the authority granted in the Virginia Coal Surface Mining Control and Reclamation Act of 1979 (Chapter 19, Article 4, Section 45.1-260 et seq.,) with funding provided by the 2010 Consolidated Grant, has determined and found that:
Land located on Shortt Town Road on the west side of Rt. 632 (Right Fork Road) in Wise County, Virginia, has been adversely affected by past coal mining practices, to wit:
1. Property owners, both known and unknown, are hereby notified of the Division’s intention to reclaim the area adversely affected by past coal mining practices. The purpose of this Notice is to provide notice to any landowners, heirs, assigns and devisees who may be affected by the proposed reclamation and who have not been identified and who have not granted written consent.
2. The public health, safety and property is endangered by: drainage from an abandoned surface mine.
3. The proposed action consists of:
Construction and installation of drainage structures to capture and convey mine drainage to Mullins Fork.
Regrading and resurfacing affected areas of Shortt Town Road.
Revegetating all areas disturbed by this work.
Utilizing standard erosion and sediment control measures for the duration of this project
Traffic on Shortt Town Road will be interrupted periodically.
